INSTRUCTIONS: Please complete this questionnaire to assist the Student Assembly Board of Directors in its selection process for the CPI/CSIF Student Assembly Liaison position. If you have any questions, please email student-assembly@apta.org.

 

Position  Description: The CPI course provides a standardized training program and assessment designed to evaluate student clinical performance. The CSIF is used to collect information from clinical education sites. The student liaison will attend quarterly advisory meetings with the CPI/CSIF Board to provide valuable input from the student perspective.

 

DEADLINE: Please complete this questionnaire and submit via email to student-assembly@apta.org no later than  at 11:59pm, EST.  Late submissions will not be considered.
