A member of this TF will be led by a TF Chair to develop deliverables available to physical therapists, physical therapists’ assistants, students and the client’s these groups serve. KT deliverables should use the CPG: PT Management of individuals with Parkinson disease to:
· Translate findings into the Geriatric 5Ms model
· Integrate the concept of the 5Ms with CPG recommendations
· Position the best evidence in the PT Management of individuals with Parkinson disease to multiple stakeholders via national and international presentations and webinars
· Develop a plan to measure the success of creating these materials
· Assist clinicians to implement best evidence at a personal level in their clinical practice,
· Assist practitioners to implement best evidence within a clinical environment or organization that include interdisciplinary and interprofessional care teams
· Assist the community of adults living with Parkinson disease implement best practice for the management of the disease process
Participants in the KT TF should have knowledge or experience (in at least 2 or 3 of the following):
· With the current best evidence for the physical therapist management of individuals with Parkinson disease
· Applying the 5Ms concepts to different clinical populations
· Developing knowledge translation products
· Practicing in clinical settings where they serve older adults with Parkinson disease
· Teaching clinical content to entry-level students
